Interdisciplinary surgery of anal carcinoma. A case report

After radiochemotherapy the treatment of persistant anal carcinoma requires radical abdominoperineal resection (APR). Primary wound closure often results in perineal wound complications necessitating revision. Simultaneous primary reconstruction of the perineal wound has been shown to reduce the incidence of wound complications as well as the length of hospital stay. To prevent surgical complications after APR a close interdisciplinary collaboration with plastic surgeons is mandatory to allow effective reconstruction, which may decrease local recurrence and morbidity. A case report of a patient with persistant rectal carcinoma is presented where APR and primary perineal wound reconstruction with vertical rectus abdominis muscle (VRAM) was carried out. The patient was released soon after an uneventful postoperative course.
