Aim: Masson tumor can be seen in any blood vessel in any region of the body but it most frequently is seen in head-neck region often with an intradermal or subcutaneous location. It can be mixed with angiosarcoma. Herein we present a case of intravascular papillary endothelial hyperplasia (IPEH) localized in the dorsum of the hand. Methods: Thirty four years old female presented with a painless mass on the dorsum of her left hand. The mass was resected totally. It was seen as a part of the regional vascular structure and was in red color and ellipsoid shape with macroscopic dimensions of 1x0.6x0.3cm. There were no postoperative complications and no recurrence of disease at 1 year follow-up. Findings: Microscopic examination revealed intraluminal recanalized thrombus and numerous papillary structures. The papillary structures were covered with endothelial cells and had homogenous eosinophilic core. There were no mitotic or atypical cells. There was no necrosis. These findings led to the diagnosis of IPEH and thrombus. Conclusion: IPEH should be considered as a differential diagnosis in masses which seem like continuation of the regional vascular structure in the hand and fingers. Contrary to angiosarcoma, simple total excision can lead to a complete cure in this entity.
