Neuroleptic-induced akathisia (NIA) is the most common form of extrapyramidal symptoms (EPS). It is often refractory to treatment and its presence may preclude adequate trials of antipsychotics. Two patients, with other indications for bupropion (BUP) therapy, demonstrated moderate to severe NIA which responded dramatically to BUP. This improvement was sustained. These serendipitous observations require replication, but further study appears warranted. As BUP lowers the seizure threshold, additional studies must be undertaken with great caution.
