Functional mitral regurgitation conveys adverse prognosis, doubling mortality after myocardial infarction; it is caused by an imbalance of the entire mitral-ventricular apparatus. A failure in any part of this complex in a patient with coronary artery disease is named "ischemic mitral regurgitation". Echocardiography represents the most useful tool in establishing the diagnosis but other techniques to demonstrate myocardial viability are necessary. Optimal treatment is intensely debated and will be difficult to resolve due to the large number of incompletely effective surgical strategies and the lack of randomized trials. The surgical approach has evolved from revascularization alone to additional mitral valve procedure showing controversial results. We aim to review most essential principles and surgical options to clarify the best management.
