Hip Pain and Sacroiliac Joint

The pain of the hip does not always have its origin in an affection of the hip joint: discrepancy between subjective complaints and x-ray findings of coxarthrosis. Reference to the studies of GUTMANN who frequently found painful coxarthrosis with a bad prognosis in case of horizontal positioning of the sacrum. The author describes pain of the hip (in normal x-ray findings) often as the only symptom of a lumbar or ileosacral lesion. On the other hand he found at the same time the existence of a bending contraction (loss of hyperextension) of the hip joint and a blocking of the ileosacral joint in case of an already known arthrotic hip joint with inter current pain attacks. Manipulation of the hip joint and mainly of the ileosacral joint are loosening the bending contraction and remove the pain attacks. The mutual influences between hip joint and ileosacral joint are discussed: increased muscular pressure on the joints in case of bending contraction and of horizontal sacrum (unstable sagital balance). Functional lesions of the ileosacral joint cause bending contraction and therefore increased loading of the hip joint by tension of hip muscels mainly of the iliac muscle (Keels). Finally the bending contraction of the hip joint is followed by ventral tilting of the pelvis with a horizontal sacrum and lumbar hyperlordosis.
