Periprosthetic fractures associated with total shoulder and elbow arthroplasty

摘要
Background. The increasing number of implantations of total shoulder and elbow prostheses ultimately results in an increase in the corresponding complications, such as periprosthetic fractures. Assessment and therapy selection. From the viewpoint of the authors, agreement on a reasonable classification with the possibility of deriving an appropriate therapy would be desirable, particularly in the region of the elbow joint. As for all periprosthetic fractures, the main aspect is the assessment of the remaining residual stability. Therapy. Conservative therapy is often only possibly for fissures and unicortical fractures. In cases of reduced bony substance sufficient stability can often only be achieved by replacing the prosthesis. In cases of sufficient bone quality and a firmly sited prosthesis, the general criteria for osteosynthesis are applicable. As there are only relatively few such cases and given the necessity for a comprehensive technical apparatus and experience in dealing with such injuries, treatment in an appropriate traumatology and endoprosthesis center would seem recommendable.
