Long-acting reversible contraceptives (LARCs), the intrauterine device and implant, are recommended as the first line of contraception by the American College of Obstetrics and Gynecology and the American Academy of Pediatrics for adolescents in the United States because of their low failure rate and high compliance, but the United States has high teen pregnancy rates. Barriers to more prevalent use include patient and provider misconceptions regarding infection, infertility, and insertion. Overall, the risk of adverse outcomes with LARC use is low and fertility is maintained after removal. Addressing misconceptions will lead to increased use of LARCs and decreased unintended pregnancies.
