This article presents current good practice guidelines retained by an expert working group for sedation for existential distress in adults in specific situations of home and geriatric ward care. Four questions were examined: what is the definition of sedation for distress? What are the indications for sedation? What drugs are used? What are the practical modalities for sedation in these conditions? The Formalized Expert Consensus method was applied. All recommendations were strongly supported by the experts. Sedation is a complex care procedure, difficult to implement. This complexity involves both indications for sedation and evaluation of its effect. In clinical practice, the decision to institute terminal sedation for distress raises more questions than it answers. (C) 2010 Elsevier Masson SAS. All rights reserved.
