Objective: To describe and report a uncommon pathology, that should be included in the differential diagnosis of low back pain and sciatica. Method: Report of two cases of lumbar epidural lipomatosis admitted to Neurosurgical Service, Santa Casa de Santos, Sao Paulo, Brazil, in a period of three years. Results: Following surgery the patients presented complete resolution of symptoms. Conclusion: Epidural lipomatosis should be included in the differential diagnosis in cases of low back pain and sciatica, mainly in patients under long lasting corticotherapy as well as in the obese.
