In practice, clinicians label as ADEM a wide spectrum of postinflammatory monophasic demyelinating syndromes that range in severity from site-restricted CNS insults of variable types (optic neuritis, cerebellitis, myelitis) to devastating attacks of CNS demyelination that affect the entire neuraxis. Recently developed diagnostic criteria concerning pediatric patients attempted to delineate the clinical and imaging features of ADEM, in order to achieve a united approach regarding diagnosis and therapy. In practice, ADEM presents with pleiotropic clinical phenotypes and distinction from Multiple Sclerosis often remains a conundrum. We review the literature concerning the frequency, manifestations and clinical outcome of this diverse group of syndromes that comprise ADEM and discuss the difficulties that often arise in the differential diagnosis of MS.
