Coronary artery aneurysms are a relatively infrequent anomaly of coronary circulation. We report the case of a 51-year-old male patient, with no previous history of comorbidities, who presented on admission with cardiac tamponade. Diagnostic tests showed the presence of a giant aneurysm of the right coronary artery. Aneurysmectomy was performed and the right coronary flow was established with an internal right mammary artery bypass. The natural history of this abnormality and his therapeutic management remains unclear. Surgery has shown excellent results when is correctly indicated.
