The pathogenesis of gastroduodenal ulcer disease is not limited to a single factor but can involve an entire spectrum of causes, such as gastric acid hypersecretion, insufficient mucosal protection, and pathologic duodenogastric reflux. Computerized ambulatory 24-hour gastric pH monitoring can overcome some of the technical difficulties of gastric secretion analysis. Using a sophisticated analysis, with 24-hour gastric pH monitoring it is possible to differentiate the pathophysiologic background of each ulcer (i.e., whether it is related to acid hypersecretion, pathologic duodenogastric reflux or combinations of the two). This allows for specific medical or surgical therapy.
