A young man with an infectious mononucleosis underwent splenectomy in emergency for spontaneous splenic rupture. This is a rather rare mononucleosis complication due to variation of the spleen histologic structure and to splenomegaly. TRhe prognosis is favourable when the prognosis is made on time and the right treatment immediately started. With the exception of those cases haemodynamically stabilized (lesion which corresponds to a subcapsular haematoma), splenectomy remains the treatment of choice. Conservatively treated patients must be closely followed and at destabilization first sign, it is imperative to proceed as soon as possible.
