Purpose. - Carbon monoxide (CO) poisoning is a serious condition and is a public health problem. Neurological impairment can lead to vital and functional prognosis. Methods. - A retrospective monocentric study of 19 cases of severe CO poisoning with neurological impairment, collected between 2004 and 2014. All patients received hyperbaric oxygen therapy (HOT). Results. - The poisonings were all accidental and due mainly to the defective conditions of the water heaters. Young subjects are the most affected with masculine dominance. An upsurge in poisoning during the winter period was noticed. The majority of patients presented disorders of awareness (17 cases). Headache and dizziness were common (16 cases). Other signs were observed: speech disorder, Parkinson's syndrome, memory disorder. Cerebral MRI showed lesions of the central gray nuclei (7 cases) and the cerebral cortex (5 cases). The evolution was very favorable in 9 cases with recovery of a normal neurological status, favorable in 7 cases with minimal sequelae and unfavorable in}cases. Conclusion. - CO poisoning can leave serious neurological sequelae. Only precocious treatment with HOT may limit the consequences. Active prevention measures are needed to reduce the incidence and severity of this condition. (C) 2017 Societe Francaise de Toxicologic. Analytique. Published by Elsevier Masson SAS.
