Background: Iatrogenic vascular injury during gynecologic surgery can be a serious complication. In addition to the risks of hemorrhage and death, this type of injury places patients at risk for postoperative thrombosis. Case: A patient sustained an iatrogenic vascular injury and underwent primary repair. Subsequently, the patient developed postoperative thrombosis at the site of repair. Results: The patient was then managed successfully with catheter-directed thrombolysis (CDT). Conclusions: According to current guidelines, recent surgery is considered to be a contraindication to CDT. However, the safe use of limited CDT with adjunctive mechanical thrombectomy in this case after recent surgery, suggests consideration of this option in similar cases.
