Distal Biceps Tendon Endoscopy and Anterior Elbow Arthroscopy Portal

The authors present a new technique of distal biceps tendon endoscopy and anterior elbow arthroscopy portal. Tears of the distal biceps tendon are difficult to assess by clinical examination and imaging techniques. This is important, as debridement and conservative measures may treat minor partial tears, whereas more extensive tears require surgical reattachment. Endoscopy using the bicipitoradial bursa allows assessment of the distal biceps tendons. This technique gives a clear and magnified view of the tendons without the need for a large open approach. The tendons can also be viewed as the forearm is rotated, which is not possible even with an open approach. Debridement and resection of synovitis may be performed, or progression to formal surgical exploration of a large partial tear may occur. This avoids the need for large surgical exposures of partial tears that involve less than 50% of the tendon insertion.
