Safe and Accurate Utilization of Patient-specific Guides in Total Knee Arthroplasty

Patient-specific guides (PSG) are an emerging technology in total knee arthroplasty (TKA). This technology utilizes advanced imaging modalities to create 3-dimensional models of a patient's unique anatomy. This model is then utilized in conjunction with computer software to preoperatively determine bony resection, implant sizing, and implant placement. The PSG are then utilized intraoperatively to efficiently and accurately reproduce the preoperative plan. We believe proper use of the PSG relies on proper exposure, confirming proper guide placement, and checking for accurate bony resection. All 2 of these objectives can be achieved by honoring traditional methods and utilizing traditional tools such as extramedullary guides, calipers, and styluses. In this article, we describe our preferred technique for safely and accurately performing a total knee arthroplasty using PSG.
