The neurological features of selenium deficiency are poorly understood, and deserve more recognition for early diagnosis and treatment. Here, we report a 63-year-old parenteral nutrition-dependent woman with Crohn's disease who presented with visual loss, myelopathy and macrocytic anemia. It was found that the selenium level was below the sensitivity limit in the serum. On selenium supplementation, which ameliorated anemia, her consciousness slightly improved, but vision and sensation did not. We suggest that visual disturbance and myelopathy after anemia can be warning signs of selenium deficiency.
