Early stages of asymmetric osteoarthritis of the ankle joint have been shown to benefit from the realignment surgery, and total ankle replacement has become an accepted alternative to fusion in end-stage osteoarthritis. Combination of the 2 procedures offers the possibility to replace the ankle joint even in severely malaligned hindfoot and may help to salvage malpositioned prostheses. In this study, we describe our experience with simultaneous correction of severe malalignment and replacement of the ankle joint. In addition, the technique of secondary supramalleolar osteotomies for the treatment of malpositioned implants is described.
