Ankylosing spondylitis - disease still diagnosed too late

摘要
There are several reasons for the long delay in the diagnosis of ankylosing spondylitis (AS). One of them is about 7-year delay-between first symptoms and radiological sacroiliitis. These radiological changes are essential for the diagnosis of AS acc. to the modified New York criteria. Primary care physicians ought to have easily screening parameters for an early identification of patients with suspected AS among the large group of cases with chronic low back pain. From the practical point of view the best measures are now "inflammatory back pain" (chronic back pain of > 3 months with improvement by exercise duration in patients in whom first symptoms of this pain started before the age of 45) and HLA-B27 positivity.
