Background: Hip instability can be anticipated in rare cases of primary hip replacement and is a common cause of failed total hip arthroplasty (THA). One approach to hip instability is THA using a constrained acetabular component. Methods: Twenty THAs were performed in 18 patients. A constrained acetabular liner (Trilogys Longevitys, Zimmer Inc. Warsaw, IN) was implanted to treat anticipated or prior hip instability. Results: At an average of 28.1 mo, none of the patients had experienced a dislocation of the constrained implant. Conclusions: The Trilogys constrained acetabular liner is a viable option to treat patients with recurrent dislocations or complex hip instability.
