The annual update of the German Diagnosis-Related Groups (G-DRG) system has recently been published and is summarized in this overview. Although the transition period ("convergence phase") has been prolonged to 2010, the G-DRG catalogue is already the exclusive basis for all hospital reimbursements in Germany. Important changes in the G-DRG algorithms in the year 2009 are thus described in this article, with a focus on cardiology. In addition to structural changes in the G-DRG algorithms, the article describes technical changes to the system and changes in the reimbursement of the DRGs. In summary, the G-DRG system has now developed a degree of stability concerning the classification of DRGs and related key diagnoses and procedures. The changes in the G-DRG algorithms therefore mostly concern details. In particular, payments for implantable cardioverter defibrillators (ICD) have been considerably reduced. However, a number of limitations still needed to be corrected by the end of the transition period to ensure correct reimbursement.
