The incidence of Barrett's cancer is rising rapidly yet endoscopic surveillance is still controversial because of its low detection rate and poor cost-effectiveness. Markers of high risk independent of dysplasia are currently being sought to improve surveillance. Promising new markers are-being assessed; aneuploidy, p53, adenomatous polyposis coli gene, c-erb B2 and the epidermal growth factor receptor have potential, but prospective studies are required.
