Early initiation of appropriate empiric antimicrobial chemotherapy has central prognostic importance in patients with community-acquired pneumonia. When choosing an antibiotic regimen, clinical, demographic, and epidemiological information to identify risk factors for specific pathogens as well as antibacterial and pharmacodynamic characteristics of the antibiotic drug are important to consider. This article discusses key aspects of clinical decisions on optimal antibiotic treatment and draws conclusions for empiric antimicrobial therapy in patients with community-acquired pneumonia in the outpatient, hospital, and intensive care settings.
