Degenerative lumbar scoliosis is a common condition, with a higher prevalence in older populations, that can lead to substantial pain and disability. After a course of nonoperative treatment, surgery is indicated to address neurological deficits and/or disability resulting from deformity with or without stenosis. Surgical intervention may include decompression with or without fusion and can be performed from an anterior, lateral, posterior, or combined approach. Despite a relatively high prevalence of perioperative complications, the overall long-term outcomes of surgical interventions for degenerative lumbar scoliosis are favorable in well-selected, medically optimized patients.
