Background Hypertension is a key risk factor for the development of cardiovascular diseases and one of the leading causes of death worldwide. Existing guidelines for the management of hypertension highlight the advantages of self-home blood pressure monitoring, and the current literature supports the view that home pressure measurements contribute to the improvement of cooperation with the patients and the effectiveness of antihypertensive therapy. The aim of the study was to collect data on patient's daily habits of home blood pressure control and to assess the relationship between conducted in the home BP control, and the effectiveness of treatment of patients with hypertension and high cardiovascular risk. Materials and methods The study was conducted in a population of adult patients with diagnosed hypertension observed between the two visits W1 and W2, between which participants had engaged in self-monitoring of blood pressure at home. Totals for analysis included 6656 patients. Conclusion Most patients require polytherapy to the achievements of proper control of blood pressure. The study has not been shown to lead logbook at home significantly contributed to improved blood pressure control.
