Allergic Disease and Reactions to Food in Patients with Inflammatory Bowel Disease

Adverse reactions to food (ARF) in patients with inflammatory bowel disease (IBD) are known to be more common than in people without IBD; however, the prevalence of IgE-mediated food allergy is unclear. To check whether the immune-mediated food allergy occurs more frequently in IBD than in control populations, we have performed a questionnaire in 1430 patients with IBD and 175 controls, which supported the assumption that patients with IBD suffer more often from allergies, especially food allergies than controls. In a second, detailed study of 237 patients with IBD and 173 controls, we could confirm that ARF and probably food allergy occurs at higher rates in IBD, especially in Crohn's disease, less in ulcerative colitis, compared to appropriate controls. On the other hand, we could not confirm that the rate of atopic diseases or extraintestinal allergies is increased in IBD. These findings suggest that in IBD the intestines are destined to exert an allergic reaction, possibly because of an impaired intestinal barrier. On the other hand, allergic diseases per se are not more frequent in IBD than in controls.
