Background: Prolactin is a versatile hormone with different functions for reproduction, sexuality, metabolism and immunity. The neuronal control of its secretion is complex and involves different hypothalamic nuclei. Animal studies were able to uncover more than 300 functions of this peptide; however, the physiological role for human sexuality and reproduction is only partly known. Objective: Presentation of the fundamental mechanisms of prolactin for sexual function and reproduction. Material and methods: Research of the literature, selection and discussion of high-quality publications. Results: This review article presents the fundamental mechanisms of prolactin production and secretion and explores specific secretion patterns with respect to sexuality and reproduction. Own studies have shown a pronounced and long-lasting prolactin secretion in men and women after orgasm as well as a possible alteration of the prolactin secretion pattern more than 48h after sexual intercourse in women. Apart from reproductive functions prolactin may also have a role for sexual satiation, satisfaction and bonding. Finally, we discuss the significance of prolactin in clinical situations, such as hyperprolactinemia and drug-induced changes of prolactin levels. Conclusion: Prolactin is a versatile hormone with various properties in sexual physiology and reproduction and alterations have a high clinical relevance; however, more ongoing research is necessary in the future. © 2017, Springer Medizin Verlag GmbH.
