Background: Stemless anatomic or reverse shoulder arthroplasty is a surgical procedure used to treat various shoulder conditions, including primary or secondary osteoarthritis, irreparable rotator cuff lesions and rotator cuff-deficient shoulder osteoarthritis, and chronic instability. Compared to conventional stemmed shoulder arthroplasty, stemless shoulder arthroplasty offers several theoretic advantages, including shorter operative time, bone preservation, and fewer perioperative complications like humeral shaft fractures or stress shielding. Due to the bone preservation and the natural strength reduction with undamaged elasticity of the epiphysis and diaphysis, long-term osteological benefits result.Methods: This review focuses on the existing literature regarding return to sports after stemless total anatomic or reverse shoulder arthroplasty.Results: On the background of a significantly increasing number of shoulder arthroplasties, current studies indicate a tendency toward improved range of motion and significant enhancement in the Constant-Murley score and sporting activities for both anatomic and reverse shoulder arthroplasty. The individual's preoperative activity level and the type of prosthesis used are crucial factors influencing the return to sports. In addition, patients' expectations regarding return to sport after reverse shoulder arthroplasty are increasing.Conclusion: This review of the current literature regarding return to sports after stemless anatomic or reverse shoulder arthroplasty highlights the pronounced differences between anatomic and reverse procedures.
