Psoriatic arthritis is a disease condition which is seen in patients with psoriasis. There are various other medication and other causes which are associated with psoriatic arthritis. A young male was admitted in the hospital with chief complaints of multiple joint pain from 2 to 3 days, breathlessness, and abdominal pain on and off and chills on and off in the last 15 days. The patient is a known case of psoriasis from last year and had a knee pain from a few years and for that he was taking NSAID as over the counter (OTC) medication. Patient was found to have an abnormal X-ray and started bone destruction. The patient is a known case of psoriasis from last year and he was taking NSAID for knee pain, which is a risk factor for developing psoriasis; so it is a confirmed case of psoriatic arthritis. So, to manage this condition, the patient was prescribed with various medications like Methotrexate, Sulphasalazin, and Tramadol during hospitalization. In this case, the pharmacist has the main role since the patient was taking medication for knee pain as OTC basis; so while prescribing any OTC medication, the pharmacist has to check for which condition the patient is taking medicine. Early detection and prompt differential diagnosis is required to start appropriate therapy in patient. There is a need to counsel the patient on OTC medication to avoid future complications associated with other disease conditions.
