Cardiorespiratory polysomnography (PSG) comprises EEG (2 leads), EOG, EMG (chin/legs), ECG, airflow (mouth/nose), respiratory effort (thorax/abdomen), O2-saturation, body position, and video imaging. Variables are continuously registered and documented, providing information on and allowing the analysis of the duration and depth of sleep as well as breathing disorders (apnea-hypopnea index, obstructive apneas, respiratory related arousals, O2-desaturation). PSG aids in the diagnosis, differential diagnosis, therapy, and therapy control of sleep related diseases according to ICSD/ICD-10. It is, as a rule, performed on an inpatient basis. A sleep medicine diagnosis and differential diagnosis are prerequisites. The clinical algorithm from the S2 guidelines "non-restorative sleep" as well as standard, guideline, and option-based evidence determine the procedure. A directive by the Joint Federal Committee on the development of new therapies specifies the in- and outpatient procedures. A clear diagnosis of moderate to severe obstructive sleep apnea syndrome is indicative for nasal continuous positive airway pressure therapy. © Springer Medizin Verlag 2007.
