Im Bereich der elektiven kolorektalen Chirurgie gab es in den letzten Jahrzehnten erhebliche Fortschritte mit zunehmender Modernisierung der Operationstechniken und Implementierung eines guten perioperativen Managements der Patienten. Trotz dieser Fortschritte treten immer noch postoperative Komplikationen auf. Die Frage, inwieweit eine optimale Darmvorbereitung zum Erfolg der Operation beitragen kann, ist nicht neu. Nachdem die Darmvorbereitung vor über 50 Jahren eingeführt wurde, kam es gegen Ende des 20. Jahrhunderts zu einem Wandel bis hin zum teilweise völligen Verzicht auf die Darmvorbereitung. Neben der unumstrittenen perioperativen systemischen Antibiotikaprophylaxe wird aktuell der additive Einsatz einer oralen Antibiose oder selektiven Darmdekontamination mit und ohne mechanische Darmspülung kontrovers diskutiert mit der Frage, welche Kombination am meisten Erfolg verspricht, um die Rate an postoperativen Wundinfekten und Anastomoseninsuffizienzen zu verringern.
