ZusammenfassungDiabetes mellitus ist eine komplexe Erkrankung mit zunehmender Pravalenz. Oft bleibt eine diabetische Stoffwechsellage unerkannt. Bei der Versorgung entsprechender Patienten in Notfallsituationen muss differenzialdiagnostisch nicht nur an die hypo- und hyperglykamischen Komplikationen eines bekannten Diabetes und seine Langzeitfolgen gedacht werden, sondern auch an die evtl. untypische Diabeteserstmanifestation. Daruber hinaus sind Diabetespatienten durch kardiovaskulare, chirurgische und infektiologische Komplikationen bedroht. Besondere Risikogruppen sind Kinder, Schwangere oder geriatrische Patienten mit ihrer in Notfallsituationen oft unspezifischen Symptomatik. Die Versorgungskette sollte aus ambulanter hausarztlicher Betreuung, Rettungsdiensten und der Behandlung in Notfallzentren bestehen. Eine enge Vernetzung ist notwendig, um sektorenubergreifende und patientenzentrierte Strategien zu gewahrleisten. Die klinische Notfallmedizin sollte hier Schnittstellenfunktion haben. AbstractDiabetes mellitus is acomplex disease with rising prevalence. Many patients with diabetes are assumed to be undiagnosed. Emergency treatment of patients with diabetic disorders may be challenging: Not only must the differential diagnoses include hypo- or hyperglycaemic states and complications of apre-existing diabetes but also primary manifestations that might present atypically. Furthermore, the diabetic patient is not only threatened by cardiovascular diseases but also by surgical and infectious complications. Children, pregnant women and elderly patients are at special risk due to their atypical symptoms or missing prodromal states. Family physicians, emergency services and emergency departments should work hand in hand to ensure state-of-the-art treatment in apatient-centred manner. Emergency physicians can play akey role in connecting those sectors with each other.
