Selective amygdalohippocampectomy versus standard temporal lobe resection in patients with mesial temporal lobe epilepsy and unilateral hippocampal sclerosis: A multicenter transborder comparison of outcome

For decades, epilepsy surgery has been established as first-line treatment option in drug-resistant cases with temporal lobe epilepsy (TLE) and hippocampal sclerosis. Various methods are available in order to resect the mesial temporal lobe tissue including the hippocampal formation. To our best knowledge, our two studies that were chosen for the Alfred-Hauptmann Prize were the first ones that addressed the methodological problem to assess differing effects of these two surgical approaches the way we did. The aim of both publications was to compare selective amygdalohippocampectomy and standard anterior temporal lobe resection concerning postoperative seizure and neuropsychological outcome in patients with mesial TLE and unilateral hippocampal sclerosis. Patient groups were homogenous. The surgical method was determined only by the standard used in Freiburg and Strasbourg, respectively, and thus was not influenced by any additional methodological bias. Results show no clearly superior approach but outcome differences that may be important for the individualized prognostic information to candidates prior surgery. © 2015, Springer-Verlag Berlin Heidelberg.
