Die Kombination aus Positronenemissionstomographie (PET) und Computertomographie (CT) ist bei Kopf-Hals-Tumorpatienten in einigen Indikationen bereits ein etabliertes Verfahren, z. B. zur Primariusdetektion bei Cancer-of-Unknown-Primary(CUP)-Syndrom oder auch nach primärer Radio(chemo)therapie zur Evaluation des Therapieansprechens, insbesondere auch hinsichtlich des Nodalstatus. In manchen Fällen kann bei PET-negativem Befund so eine Salvage-Neck-Dissection vermieden werden. Im Rahmen der Primärdiagnostik bei bekanntem Primarius kann die PET-CT-Bildgebung v. a. zur Evaluation einer Fernmetastasierung eingesetzt werden. In der lokalen Ausdehnungsbestimmung bei Erstdiagnose hat die PET-basierte Bildgebung gemäß aktuellen Leitlinien (noch) keinen Stellenwert. Eine Herausforderung ist aktuell die noch eingeschränkte Kostenerstattung durch die Krankenkassen, welche sich auf bestimmte Indikationen beschränkt, und die noch fehlende flächendeckende Versorgung.
