Kidney biopsy has an important role in care and treatment of kidney transplant patients. It is by now a standard procedure used in early stages for determining deficient primary function rising creatinine levels. Kidney biopsy is needed during later disease stages in case of acute or continued creatinine rise and above all clinical symptoms such as recurring proteinuria or erythrocyturia. It is now also used intraoperatively (so-called zero biopsies) for assessing the success of a kidney transplant. Kidney biopsy has also proven particularly useful in light of the shrinking supply of donor organs and the resulting need to employ less than optimal transplant material under certain circumstances. So-called protocol biopsies are also done in many treatment centers, i.e. biopsies that are not urgently indicated but are used a certain time after transplantation to determine early subclinical rejection and changes indicating chronic organ damage. It also allows significant scientific research in kidney transplantation as a whole. © 2010 Springer-Verlag.
