Topical fluticasone: An effective corticosteroid with minimal systemic effects

Fluticasone is a moderately potent corticosteroid that has shown clinical efficacy similar to that of other corticosteroids (including betamethasone dipropionate, hydrocortisone butyrate and betamethasone valerate) in patients with atopic dermatitis or psoriasis. The majority of adverse events associated with topical fluticasone are local cutaneous reactions (most commonly pruritus). Importantly, fluticasone has a low potential for systemic toxicity, probably related to the high affinity for the glucocorticoid receptor, high lipophilicity and low systemic bioavailability of the drug. Once-daily application of fluticasone has a clinical efficacy similar to that of the twice-daily regimen in patients with atopic dermatitis, which offers potential advantages in terms of convenience, compliance and tolerability.
