Unzählige ehrenamtliche Hilfsiniativen für notleidende Flüchtlinge dokumentieren eindrücklich die hohe Prävalenz altruistischen Verhaltens in der Bevölkerung. Paradigmatisch für altruistische Entscheidungen in der Medizin ist die Organspende. Jüngste neurowissenschaftliche Befunde zeigen, dass insbesondere Empathie-induzierter Altruismus in der motivationalen Architektur des sozialen Gehirns repräsentiert ist. Seine Ursache hat dies in der sozialen Evolution von Kooperativität und Parochialität, deren Einfluss auf das Individual- und Kollektivverhalten an den Zügeln des evolutionär hochkonservierten Peptidhormons Oxytozin reguliert wird. Aus psychiatrischer Sicht imponiert Altruismus als dimensionales Verhaltensmerkmal, das bei extremer phänotypischer Ausprägung pathologische, selbst- und fremdschädigende Formen annehmen kann.
