Für die meisten war die Gicht lange Zeit nur die Folge eines ungesunden Lebensstils, ein Sinnbild für Völlerei und Maßlosigkeit und damit letztlich auch selbstverschuldet. Dieses negative Image führte dazu, dass die Therapie oft nicht konsequent erfolgte und die sozioökonomischen Folgen dieses Krankheitsbildes lange unterschätzt wurden.
