Chronic subdural hematoma (CSDH) is a neurological disease that always affects the elderly. Burr hole drainage is a relatively simple procedure and frequently used in the treatment for CSDH. Brainstem hemorrhage is a rare complication of CSDH drainage but with poor prognosis. We describe a case of 60-year-old man presenting with brainstem hemorrhage after receiving burr hole drainage of CSDH. The literatures were reviewed, and the possible mechanisms were discussed. Rapid brain shifting and hyperemia after operation may participate in this pathological procedure. Slow decompression of hematoma with steady lowering of intracranial pressure may help to reduce the complication.
