Merkel cell carcinoma is an aggressive neuroendocrine tumor with high rates of local recurrence, micro- and macrometastasis. Its poor prognosis underscores the importance of determining effective treatment protocols. However, given its rarity, there is a paucity of prospective randomized trials to guide evidence-based management. Current guidelines are largely based on retrospective reviews and expert consensus opinion. In general, treatment centers on addressing the primary tumor first, followed by management of nodal and/or metastatic disease. This review discusses therapeutic options by disease stage and highlights where questions remain to be answered. © 2015, Springer Science+Business Media New York.
