Patienten haben grundsätzlich das Recht, eine medizinische Behandlung abzulehnen. Dem Respekt vor der Autonomie des Patienten wurde in den USA 1991 durch den Patient Self-Determination Act Rechnung getragen, der die Aufklärung stationärer Patienten über das Recht auf Ablehnung einer Therapie und auf die Abfassung einer entsprechenden Verfügung vorschreibt. In diesem Zusammenhang haben die sog.„Do-not-resuscitate orders“ (DNR-Verfügungen) zunehmend an Bedeutung gewonnen. Bei schwer kranken Patienten mit DNR-Verfügungen können Operationen erforderlich werden, die überwiegend einen palliativen Charakter haben und einer Verbesserung des Befindens und der Erleichterung der Pflege dienen.
