In assisted reproduction technology (ART) cycles, luteal phase irregularities are induced during controlled ovarian hyperstimulation with gonadotropins and gonadotropin-releasing hormone (GnRH) agonists and antagonists. They can be treated using human chorionic gonadotropin (hCG) for luteal phase support or progesterone for luteal phase supplementation. The gold standard is vaginal administration of progesterone to prepare the endometrium without subjecting the patient to the side effects caused by systemic therapies. Successful luteal phase substitution includes early administration, no later than at the time of the embryo transfer, and continuation until an intact pregnancy can be verified by ultrasound. © 2008 Springer Medizin Verlag.
