摘要：
Die steigende Anzahl an Primärimplantationen von Gelenksendoprothesen führt auch zu einer steigenden Zahl an septischen Revisionseingriffen. Hierbei konnten zahlreiche patientenassoziierte und operationsbedingte Risikofaktoren für Protheseninfektionen verantwortlich gemacht werden. Nach wie vor gilt es, durch streng steriles, zügiges Arbeiten die Möglichkeit einer intraoperativen Keimbesiedlung so gering wie möglich zu halten. Bestehende Erkrankungen wie Diabetes und Adipositas sollten so günstig wie möglich präoperativ beeinflusst werden. Nikotinkarenz ist für ein bis zwei Monate präoperativ und bis zum Abschluss der Wundheilung zu fordern. Intraoperativ gehört die single-shot Antibiose mittlerweile zur klinischen Routine. Verlängerte perioperative Antibiotikaprophylaxe zeigt hierbei jedoch kein vermindertes Infektionsrisiko.
