Die Meniskuschirurgie stellt eine komplikationsarme Chirurgie dar. Generell können intra- von postoperativen Komplikationen unterschieden werden. Bei den intraoperativen Komplikationen kann es zu Verletzungen gesunder intraartikulärer (z. B. Knorpel) und extraartikulärer Strukturen (z. B. Gefäße, Nerven) kommen. Der Verletzungsprävention kommt durch adäquate Lagerung, Portalanlage und dem Nutzen geeigneter Instrumente eine Schlüsselrolle zu. Der Artikel fasst die möglichen Lösungsansätze bei intraoperativen Komplikationen zusammen. Bei den postoperativen Komplikationen stehen der Gelenkinfekt, Zysten/Ganglienbildungen, die frühe postinterventionelle Chondrolyse, die Osteonekrose der Femurkondylen und das chronische regionale Schmerzsyndrom im Vordergrund. Hier werden ebenfalls die aktuellen Interventionsmöglichkeiten bei den beschriebenen Komplikationen erörtert, um die Funktion des Kniegelenks wiederherstellen zu können.
