Our knowledge of bladder pain syndrome has evolved over the past century in ways that will make understanding the problem and finding a cause, treatment, and (we hope) cure much more likely. What was originally an "ulcer" in the bladder later became a diagnosis dependent on a combination of symptoms and an abnormal endoscopic appearance of the bladder wall, and now is considered a pain syndrome sometimes associated with other pain syndromes not involving the urinary tract. How did this happen and what does it mean? This article addresses these questions with a focus on current international guidelines, and gives the reader a perspective from which to read the newest literature and research. © 2011 Springer Science+Business Media, LLC.
