In the last 20 years intensified conventional insulin treatment (ICT) using either multiple injections or insulin pumps has become the standard treatment in patients with type 1 diabetes. As a result of large randomized controlled trials this treatment has gained the highest level of evidence in treatment guidelines today. The goal of intensified insulin treatment is to substitute the physiological insulin requirement in patients with type 1 diabetes. Basal- and mealtime-specific insulin requirements are considered, calculated and substituted individually. In addition to insulins with varying pharmacokinetics, structured teaching programs and blood glucose self-control are prerequisites of successful application of this therapy. The success of the treatment depends on how well it matches the needs of the patient. © 2008 Springer Medizin Verlag.
