In congenital ptosis with poor levator function frontalis sling is the method of choice. Different homolog and autolog materials have been used. Complications like infection or extrusion are well known. Between 1990 and 2000 42 patients with congenital ptosis and poor levator function were operated with frontalis suspension. 39 patients had Goretex® sling and 3 patients with Mersilene® suture. In all patients same technique was used. The suture was fixed to the tarsus and fixed in pentagonform to the frontalis. The aquired lidposition should be 10 before 2, due to cornea. Lidclosure layer to layer was performed and antibiotic ointment for 1 week was done. In all patients good lidposition was achieved, no infection postoperativly was found. In 3 patients with Goretex suspension and in 1 patient with Mersilene sling an extrusion occured within 3-6 months and the sling had to be replaced. Different materials have been used in the last years. No material is without any complication. Goretex® seems to be a save material in combination with exact operation technique, it is a safe and efficient method in frontalis suspension.
