Preventive oral care in a patient suffering from Epidermolysis bullosa - a case report

Introduction: Epidermolysis bullosa (EB) is a genetic disorder which causes blistering on the skin and mucosa. The blisters occur due to the minimal trauma of the soft tissue. The expression of symptoms varies depending on the different types and severities of the disease, while the oral cavity is affected frequently. The extensive impairments that result from EB necessitate an interdisciplinary medical care. A healthy oral cavity is a basic requirement in ensuring patients' ingestion and dental teams therefore play an important role in the treatment of EB. Case report: The present case report concerns a 27-year-old female patient suffering from junctional Epidermolysis bullosa (EBJ). The disorder led to recurrent blistering on the oral mucosa as well as to generalized enamel hypoplasia which required the early crowning of all of the teeth. It was considered which preventive dental measures are feasible in the patient and which specifics have to be observed. In the case report, a professional dental cleaning was performed with manually operated instruments and with a low abrasive air-polishing device containing glycine. The dental findings, the reactions of the oral soft tissue and the perception of the patient were documented. Epicrisis: The preventive dental measures could be performed widely without complications. In particular, the use of the air-polishing device did not cause any blisters and was not described as painful or unpleasant by the patient: only the temporary application of the suction tip caused a lager lesion of the buccal mucosa. The restoration with ceramic crowns proved to be effective in protecting the formerly hypomineralized and abraded teeth from caries. Conclusion: The preservation of functioning dentition in patients with EB is possible if restorative dental measures are conducted in an early stage and if oralhealth prevention is carried out in a risk-Adapted frequency. However, the vulnerability of the mucosa varies according to the type and severity of the disease so that the stated recommendations for dental treatment cannot be transferred to all EB patients.
