Fibrin monomers in acute obstetrical situations

摘要
Since 2013, we have been using the fibrin monomer assay in the hematology laboratory of the Brest university hospital. This early marker of coagulation activation allows us to early detect disseminated intravascular coagu-lation (DIC) and to monitor its evolution using a fibrin monomer/-D-dimer ratio. The assay of fibrin monomers is key in several clinical settings. This assay is complementary to the D-dimers measurement. Through two clinico-biological situa-tions, we will show the interest of these parameters in two acute obstetrical situations. The first case deals with a postpartum hemorrhage complicated by a disseminated intravascular coagulation. In this situation fibrin monomers were able to rapidly detect the coagulation activation. The second clinical case deals with the management of a placenta percreta complicated by a disseminated intravascular coagulation, for which situation fibrin monomers were an aid in the surgical procedure.
